The online forms can be downloaded using
Adobe Acrobat Reader.

All forms must be filled out completely, and mailed or faxed (510) 642-7589 to The Bancroft Library. Payment must accompany form.

Photographic duplication form
Manuscript photocopy form
Printed material photocopy form
Permission form (publications, reproductions, displays)
Requesting permission to publish manuscript materials
Policy concerning reproduction of manuscripts for placement in other institutions
